Rehabilitation Assistant

An opportunity has arisen for a motivated and ambitious Rehabilitation Assistant to join our team.


About the Role

We are looking for a compassionate, organised, and proactive Rehabilitation Assistant to join our growing Case Management team.

 

This is an exciting opportunity to work alongside experienced Clinical Case Managers and rehabilitation professionals, supporting individuals with complex rehabilitation needs to achieve their goals and maximise their independence.

The successful candidate will play a key role in supporting the Case Manager with coordinating rehabilitation programmes, supporting clients, monitoring progress, and ensuring the smooth delivery of high-quality, person-centred rehabilitation services.

The role combines direct client support, case coordination, and operational assistance within a multidisciplinary environment.

 

What You’ll Be Doing

Client Rehabilitation Support

  • Support the implementation of individual rehabilitation programmes under the guidance of Case Managers.
  • Support the Case Manager with research, such as local Gyms, Taxi accounts, Gardening, Cleaning Services.
  • Monitor progress and provide updates to the wider rehabilitation team.
  • Help identify barriers to progress and raise concerns appropriately.
  • Promote independence, wellbeing, and choice in all interactions.
  • Support clients to access community resources, education, employment opportunities, and social activities where appropriate.

 

Case Coordination

  • Act as a key point of contact for clients, healthcare professionals and external agencies.
  • Coordinate appointments, assessments, treatments, and community-based services.
  • Maintain accurate and confidential case records, documentation, and reports.
  • Assist with the review and updating of rehabilitation plans.
  • Attend multidisciplinary meetings and contribute relevant information to support effective decision-making.
  • Attend client meetings with the Case Manager

 

 

Supporting the Head of Case Management

  • Maintain oversight of active cases and update case tracking systems.
  • Monitor review dates, action plans, and key deadlines.
  • Support referral allocation and assist with caseload management.
  • Attend meetings and record actions.
  • Liaise with therapists, support workers, insurers, solicitors and other stakeholders to ensure effective communication and follow-up.
  • Assist with monitoring rehabilitation budgets, invoices, and expenditure.
  • Provide administrative support to the Head of Case Management and wider team.

 

Service Improvement

  • Support policy and procedure reviews.
  • Assist with quality assurance activities and service audits.
  • Gather and analyse client feedback to support service development.
  • Help maintain databases of approved providers and community resources.
  • Contribute to projects that improve service quality and efficiency.

 

What We’re Looking For

Essential

  • Experience within rehabilitation, healthcare, social care or a related support role.
  • Excellent organisational and coordination skills.
  • Strong communication skills, both written and verbal.
  • Ability to work independently whilst also being an effective team member.
  • Experience maintaining accurate records and handling confidential information.
  • Good IT skills, including Microsoft Office and electronic record systems.
  • A genuine commitment to delivering person-centred support.

 

Desirable

  • Qualification in Health & Social Care, Rehabilitation, Occupational Therapy Support, Physiotherapy Assistant, or a related field.
  • Experience supporting individuals with neurological, physical, or complex rehabilitation needs.
  • Knowledge of rehabilitation pathways and community-based services.
  • Full UK driving licence and access to a vehicle and be able to travel a 2-hour radius at times to support client visits/ hospital visits.
